ADDITIONAL EQUIPMENT, SYSTEMS & SERVICES DISCLAIMER NOTICE

The undersigned Customer acknowledges that a representative of St. Louis Alarm Company, LLC (hereinafter referred to as “St. Louis Alarm Co.” or “Alarm Company”) has explained that additional equipment, systems, services, and levels of protection may be available for an additional cost.

Customer further acknowledges that they have been given sufficient opportunity to review, ask questions, and consider these additional options, and have elected not to purchase or include such additional equipment, systems, or services at this time.

The additional equipment, systems, and services discussed may have included, but are not limited to:

  • Additional points of protection
  • Roof, attic, wall, and exterior protection
  • Hardwired systems
  • Independent or secondary backup systems
  • Wireless systems
  • Access control systems
  • Additional door/window contacts
  • Fire, smoke, carbon monoxide detection
  • Water, heat, and temperature monitoring
  • Motion detectors
  • Video surveillance (CCTV)
  • Audio surveillance
  • Sprinkler monitoring systems
  • Video monitoring and verification services
  • Data storage and retrieval services
  • Guard response services
  • Cellular backup communication
  • Radio backup communication
  • Dedicated telephone line communication
  • UL-certified installation
  • UL-certified monitoring services
  • On-site or stationary guard services

Customer understands that declining any of the above options may reduce the overall effectiveness and level of protection provided by the system.


COMMUNICATION METHOD DISCLOSURE

Customer further acknowledges that St. Louis Alarm Company has explained the differences between communication methods, including:

  • Traditional telephone (POTS) lines
  • Cellular communication
  • Internet/VoIP (Voice over Internet Protocol)

St. Louis Alarm Company recommends the use of cellular or dedicated communication methods for increased reliability.

Customer understands that VoIP (internet-based phone service) may:

  • Be less reliable than traditional or cellular communication
  • Be subject to internet outages, power loss, or network failures
  • Not always be fully compatible with alarm systems

☐ Customer elects to use VoIP/internet-based communication

By selecting VoIP, Customer acknowledges and agrees that such use is at Customer’s sole risk, and St. Louis Alarm Company shall not be responsible for communication failures, delays, or signal transmission issues resulting from such service.
